Apparently anti-government types in South Carolina have to register with the state. How do they enforce that?

it's one of those laws that are designed so that they have a way to prosecute those groups at any time they desire. They know the groups won't register, so when a group becomes a problem, they can just bust them on this law and not worry about putting together a full case.
